ABSTRACT

Background: In March 2020, the world health organization declared COVID-19 a world wide pandemic. Countries introduced public health measures to contain and reduce its spread. The effect of mandated societal lockdown to reduce the transmission of corona virus disease 2019 (COVID-19) on road traffic accidents is not known. For this we performed an in-depth analysis singdata of emergency and trauma centre UPUMS, Saifai. As most of the manpower was involved in managing Covid patients directly or indirectly, it was a challenge to manage these mass casualty patients who require intensive care as well as Medicolegal documentation, record keeping, Consent for life saving procedures in absence of Relatives. Material(s) and Method(s): We reviewed data on total 2876 road traffic accident records in UPUMS, Saifai from January 1, 2020 through September 30, 2020. We treated March 20th as the first day of mandated societal lock down and 1st July as the first day of re-opening. Result(s): We have found that the reis increase in road traffic accidents resulting in serious or fatal injuries during lockdown and post-lockdown period. There was increased Medicolegal burden in spite of the decreased medical resources, manpower as most of manpower and resources were being utilized for covid patients. Conclusion(s): Road traffic accidents are a prominent contributor to hospitalization and may negatively impact the existing hospital resources directed towards COVID-19.Copyright © 2023, World Informations Syndicate. ABSTRACT

SARS-CoV-2 is a pandemic that affects road traffic flaw and crashes globally. This study attempted to compare the situation of road traffic crashes in the city of Budapest before and after the SARS-CoV-2 pandemic to better understand its long-term percussive effects. The study considers 12208 road traffic crashes that registered between 20 May 2018 – 31 December 2021. The rate and severity of road traffic crashes during the SARS-CoV-2 pandemic examined by using a percentage frequency distribution and a severity index. This study depicted that most crashes reported during the normal daytime between15:01-18:00 (peak hour). The study indicated that during the SARS-CoV-2 pandemic the road traffic crashes were reduced by 20.15%. A rear-end collision was one of the most common type of catastrophes highly registered. Road users, particularly drivers, heavily endorsed crashes. Even though the proportion of road traffic crashes caused by alcohol consumption was modest (6%), the rate of alcohol consumption and its concentration increased slightly during the SARS-CoV-2 pandemic. At the same time the number of crashes caused by high-speed traffic maneuvers reduced. Improper interpretation of road traffic signs, road pavement condition and failure to respect proper sight distance were influential reasons of road traffic crashes among the top. Meanwhile, the distributional impact of careless driving in the aftermath of the SARS-CoV-2 pandemic causes a shift in rank. Therefore, this study proved that during SARS-CoV-2 pandemics road traffic crashes reduced, the rate and concentration of alcohol consumption increased, and careless driving was encouraged.

ABSTRACT

Background and Objectives: In every tertiary care hospital, casualty deals with the emergency cases of which the majority are Medico legal cases. The knowledge about the incidence of Medico legal cases is important to recognise the socioeconomic burden. The present study was conducted to scrutinize different Medicolegal cases at the emergency department of Mediciti Hospital, Medchal. The idea of the study was to find out frequency of several types of medico legal cases at casualty of Mediciti Hospital, Medchal. Material(s) and Method(s): It was a record based cross sectional study in which all the MLC cases registered in MLC record book from March 2020-March 2021 were analyzed. The data was collected on age, sex, type of Medico legal cases, road traffic accidents, mode of occurrence, month-wise distribution of medico legal cases and the time of occurrence. Results were expressed in numbers and percentages. Result(s): Out of all 355 registered medico legal cases, of which 258(73.2%) were males and 94(26.76%) were females. Maximum cases were from the age group of 20-29 years i.e., 127(43.09%). Majority of the MLC's registered we are due to road traffic accidents 144(39.7%) followed by Assault 49(13.8%), falls-43(12.7%), poisoning-43(12.1%), and injury at workplace-43(12.1%), accidental-14(3.9%), snake bite-8(2.2%) and others (7.4%). Conclusion(s): The present study shows RTAs account for a major part of MLCs. By proper education and training of safety measures among public decreases the cases. Enforcement of strict laws reduces the incidence of road traffic accidents. ABSTRACT

In order to create a sustainable future for the urban environment in s=Smart cities, it is necessary to develop a concept of urban transport, partially reduce the use of traditional transport, primarily cars, as well as the environmental pressure on society, which is essential to move to a sustainable urban future. In the latest discussions on the future of the urban transport system, the quality of the environment, and the possibility of its improvement are discussed, this issue became especially relevant with the onset of the pandemic, when the lockdowns were introduced. The problem of sustainable transport in urban areas has been recognized in academic studies, searching for appropriate models and solutions. The article presents the latest literature review and illustrates the newest trends with several examples. VOS Viewer software has been used to classify the different keywords, according to their co-citation, following clustering techniques. By analyzing the research conducted by other researchers, it has been possible to structure the ecosystem and trends in the Urban Transportation Concept, also mentioning likely future trends. Based on the literature analysis of the Sustainable Urban Transport, the authors of the study found that a large group of researchers deal with technical solutions and innovative business models, while the essential behavioral aspects are examined in less detail. Extensive literature analysis allowed the authors to select several solutions to achieve the transformation towards sustainable transportation in urban areas: new vehicle technologies and their environmental factors' analysis, geographic information systems, the analytic hierarchy process method, the time series analysis of road traffic accidents using multiplicative models, electrification and use of Friedman Analysis of Variance by Ranks, as well as innovations in sharing mobility.

ABSTRACT

The high demand and acute timeliness that characterizes instant delivery entail the challenges of high labor costs and an increase in courier traffic accidents. Autonomous delivery vehicles (ADVs) may serve as a key solution, with their attendant reduced labor input and higher efficiency. Customers play a key role in the successful implementation of ADVs on a large scale. However, understanding the factors that affect customers' intentions to use ADVs is still limited. Compared to autonomous driving, ADV customers are ultimately not the real users, who only are served by ADVs during the last leg of a trip. On account of this, the Technology Acceptance Model (TAM) may not be well-fitted for explaining the dynamics involved in ADV adoption. Within the context of ADVs, our study identified influencing factors that have not been captured by prior studies. This study incorporates infection risk, use experience, and social awkwardness into the Diffusion of Innovation (DOI) theory to explore customers' intentions to use ADVs. Data from 691 survey respondents were collected to validate the research design. The results demonstrate that compatibility, social influence, infection risk, green image, social awkwardness, and use experience all have a significantly positive impact on customers' intentions to adopt ADV services, while complexity and perceived risk both exhibited a negative impact. But no effect could be found for relative advantage, which may be because of the fact that customers only need ADVs to meet their delivery demand. This study contributes to understanding customers' adoption intentions toward ADVs, informing policymakers in formulating ADV regulations and standards, and promoting the large-scale application of ADVs in instant delivery services.

ABSTRACT

During the Great Recession in 2009, the City of Salinas began receiving requests from stakeholders concerned with the city's downtown to evaluate the area's circulation and parking. While the initial intent was to improve mobility and manage parking, the focus of the plan expanded through an extensive outreach process. Residents expressed a desire for downtown to become the heart of the city, and a place to gather for civic and community events. Through this process, the plan evolved to include four major themes: Creating a Destination Downtown, Managing Parking Resources, Building the Heart of Salinas, and Stimulating Development Activity. The process, dubbed "The Road to Vibrancy," sought to construct a plan to capture and discuss aspirations, ideas, and concepts;develop and expand concepts jointly through workshops and meetings;debate and refine the Vibrancy Plan to ensure it was done right;and to gain broad-based support so that the plan can and will be implemented. The Vibrancy Plan was shaped heavily by public input through a robust planning process. In fact, nearly all the ideas and concepts presented in the plan were discovered through dialogue with residents, property owners, business owners, and others who shared their aspirations.

ABSTRACT

Modeling traffic-accident frequency is a critical issue to better understand the accident trends and the effectiveness of current traffic policies and practices in different countries. The main objectives of this study are to model traffic road accidents, fatalities and injuries in Jordan, using different modeling techniques, including regression, artificial neural network (ANN) and autoregressive integrated moving average (ARIMA) models and to evaluate the safety impact of travel-restriction strategies during Covid-19 pandemic on traffic-accident statistics for the year 2020. To accomplish these objectives, data of traffic accidents, registered vehicles (REGV), population (POP) and economic gross domestic product (GDP) from 1995 through 2020 were obtained from related sources in Jordan. The analysis revealed that accidents, fatalities and injuries have an increasing trend in Jordan. Root mean of square error (RMSE), mean absolute error (MAE) and coefficient of multiple determination (R2) were sued to evaluate the performance of the developed prediction models. Based on model performance, the ANN models are the best, followed by the ARIMA models and then the regression models. Finally, it was concluded that the strategies undertaken by the government of Jordan to combat Covid-19, including complete and partial banning of travel, resulted in a considerable reduction of accidents, injuries and fatalities by about 35%, 37% and 50%, respectively. © 2023, Jordan University of Science and Technology. ABSTRACT

INTRODUCTION: The coronavirus disease 2019 (COVID-19) pandemic had detrimental impacts across multiple sectors of the Mexican health care system. The prehospital care system, however, remains largely under-studied. The first objective of this study was to calculate the monthly per capita rates of injury-related 9-1-1 calls, traffic accidents, and crime at the state-level (Mexico City) during the early pandemic period (January 1 through June 30, 2020), while the second objective was to conduct these calculations at the borough-level for the same outcomes and time period. The third objective was to compare monthly per capita rates of injury-related 9-1-1 calls, traffic accidents, and crime at the state-level (Mexico City) during the pre-pandemic (January 1 through June 30, 2019), early pandemic (January 1 through June 30, 2020), and later pandemic periods (January 1 through June 30, 2021). METHODS: A retrospective analysis was conducted to examine injury-related 9-1-1 calls, traffic accidents, and crime at the state-level (Mexico City) and borough-levels. Monthly per capita rates were calculated using four datasets, including Mexico City's Public Release 9-1-1 Emergency Calls, National Institute of Statistics and Geography's (INEGI) Traffic Accidents Micro-Dataset, Mexico City's Attorney General's Office Crime Dataset, and Projections of the Population of the Municipalities of Mexico, 2015 to 2030. All statistical analyses were conducted using STATA 17.0. RESULTS: During the early pandemic period, injury-related 9-1-1 emergency calls, traffic accidents, and crime experienced similar trends in monthly per capita rates at the state-level and borough-levels. While the monthly per capita rates remained constant from January to March 2020, starting in March, there was a precipitous decrease across all three outcomes, although decline rates varied across boroughs. The monthly per capita rates across the three outcomes were higher during the pre-pandemic period compared to the early pandemic period. As the COVID-19 pandemic progressed, the monthly per capita rates during the later pandemic period increased across the three outcomes compared to the early pandemic period, although they did not reach pre-pandemic levels during the study period. CONCLUSION: The precipitous decline in injury-related 9-1-1 calls, traffic accidents, and crime in Mexico City occurred at the same time as the issuance of the first wave of public health orders in March 2020. The largest decrease across the three outcomes occurred one to two months post-issuance of the orders.

Traffic-accidents are a public health problem with repercussions on population morbimortality. Objective: To analyze the impact of the pandemic on the profile of motorcycle accidents assisted at the Tertiary Hospital in 2020. Methods: Cross-sectional, descriptive retrospective study in 2017 and 2020 of 260 medical records of care for motorcycle accidents in the emergency room of the Tertiary Hospital. Statistical analysis of data and their correlations using the chi-square test (p < 0.05). Results: Of the 105 medical records in 2017, 83% are men, mean age 29.8 years, and death rate of 3.90%. Fractures in 98.10%, 64.10% exposed and predominantly the tibia (61.90%). Of the 155 medical records in 2020, 91.61% are men, mean age 31.21 years, and no deaths. Fractures in 94.84%, 37.42% exposed and predominantly the tibia (28.57%). Between 2017 and 2020, Infosiga-SP showed a relevant reduction (p < 0.001) of deaths in the hospital environment (52.46% to 31.91%). Conclusion: The incidence of motorcycle accidents increased, in-hospital deaths dropped, but the epidemiological profile of accidents at the Hospital remained unchanged. Acidentes motociclísticos configuram um problema de saúde pública com repercussões na morbimortalidade populacional. Objetivo: Analisar o impacto da pandemia por COVID-19 no perfil de acidentes motociclísticos atendidos em um hospital terciário em 2020. Métodos: Estudo transversal, descritivo e retrospectivo que analisou 260 prontuários de acidentados de moto atendidos na Urgência e Emergência do Hospital Universitário Terciário em 2017 e 2020. Realizou-se uma análise estatística dos dados e suas correlações pelo teste qui-quadrado (p < 0,05). Resultados: Dos 105 prontuários de 2017, 83% são de homens (p < 0,001), com média de 29,8 anos, e índice de óbito de 3,90%. Houve fraturas em 98,10% dos casos, sendo 58,10% expostas e predominantemente da tíbia (61,90%). Dos 155 prontuários de 2020, 91,61% são homens (p < 0,001), com média de 31,21 anos e sem casos de óbito. Houve fraturas em 94,84%, sendo 37,42% expostas e predominantemente da tíbia (28,57%). Entre 2017 e 2020, o Infosiga-SP mostrou redução significativa (p < 0,001) de mortes em ambiente hospitalar (de 52,46% para 31,91%). Conclusão: Houve aumento na incidência dos acidentes motociclísticos e queda nos óbitos intra-hospitalares, mas o perfil epidemiológico dos acidentados no hospital permaneceu inalterado. ABSTRACT

Context: Effects of coronavirus disease 2019 (COVID-19) pandemic lockdown on road traffic accidents (RTAs) in Chhattisgarh, India. Background: Most neurosurgical emergencies are from brain and spine trauma. RTA is the leading cause of such injuries. While the nationwide lockdown was an extreme measure to control the COVID pandemic, it influenced the overall road traffic dynamics and neurotrauma. Objective: This study aims to assess the impact of the lockdown on neurotrauma. Methods and Materials: This retrospective study included all patients with brain and spine injuries who were admitted between January 17th and May 31st, 2020. The study population was divided into prelockdown (PL) and lockdown (L) groups. Results: Of the 668 patients, 436 were placed in the PL and 232 in the L group. The mean ages were 36.34 (SD = 17.96) and 35.98 (SD = 16.93), respectively. Male to female ratios were 82.3:17.7 in the PL group and 79.7:20.3 for the L group. RTA-related injuries were significantly lower during the lockdown period (n = 335 PL vs. 162 L [P = 0.048]). During the lockdown, there were more mild injuries (25.91% PL vs. 36.63% L) and less severe injuries (33.25% PL vs. 18.96% L [P = 0.0002]). Mortality was significantly less (P = 0.029) during the lockdown (n = 48 L vs. 124 PL). The proportion of RTA-related neurotrauma cases increased (33.33% L1, 57.14% L2, 73.13% L3, and 80.39% L4) with each phase of lockdown (L1-L4). Conclusions: During the lockdown period, the number of trauma cases had decreased, with a significant decrease in RTA-related admissions, along with their severity and mortality. The number of trauma cases and their severity increased gradually with each phase of lockdown.

INTRODUCTION: The COVID-19 disease has given rise to various negative effects on human life in terms of health and economic and social well-being. We believe that these negative effects may have led to increased forensic incidents such as violence and suicide. Therefore, in this study, we sought to examine the effects of COVID-19 in forensic cases admitted to an emergency department. Methods: This is a retrospective observational study, performed at the emergency department of Fatih Sultan Mehmet Education and Research Hospital. Forensic cases admitted between March and June 2020 (pandemic period) and forensic cases admitted between March and June 2019 (pre-pandemic period) were compared in the study. Results: A total of 4296 patients were included in the study, of which 3011 were admitted during the pre-pandemic period and 1285 during the time of the COVID-19 pandemic. While the percentages of suicide attempts (3.6%), motorcycle traffic accidents (7.4%), and violent incidents (29.4%) were higher during the pandemic period, the percentages of in-vehicle traffic accidents (5.4%) and pedestrian traffic accidents (2.2%) were lower (respectively, p = 0.035, p = 0.005, p < 0.001, p = 0.015, p = 0.008). At the time of the pandemic, the percentages of incidents of violence against women (44.2%) and traffic accidents with a motorcycle involving men (9.3%) were higher than during the time before the pandemic (p < 0.001 and p < 0.001, respectively). Conclusions: The effects of the pandemic on our lifestyle are indisputable. This study reveals that the pandemic also affected patients who were admitted to the emergency department for forensic reasons. In addition, the increase in the percentages of suicide and violent events indicates that pandemics probably increase feelings of fear, loss, and hopelessness, and special precautions should be taken to maintain order in the society.

ContextIn the Philippines, 12,690 people died because of road traffic crashes. Every day, 35 died while three (3) are children. Stakeholders worked together to address the problem during the covid-19 pandemic. The young generation is considered the hope of our nation.ProcessThe Center for Policy Studies and Advocacy on Sustainable Development, Inc or the Policy Center, a new organization composed of the five (5) fellows of the World Health Organization- Bloomberg Initiatives for Global Road Safety tried to reconnect, reinvigorate, and reimagine injury prevention during this pandemic by addressing the situation of our children on the road. We implemented the child road safety program in two cities by involving the local leaders, communities, and politicians through organizing them and advocating through social media.AnalysisSan Pedro and Muntinlupa Cities were chosen as partners for the child road safety program because they are prone to road traffic crash as immerging urbanized cities with the increasing number of vehicles and limited knowledge on road safety. Both cities have no identified road safety leaders.OutcomesThe cities developed the Road Safety City Ordinances and Action Plans to address the need of different sectors using the five pillars of road safety with children as the center. A Road Safety module for local leaders was developed. ABSTRACT

Background With an increasing number of cases of COVID-19 disease, the Indian government announced a complete lockdown on March 24, 2020, and the country remained under complete lockdown in April 2020. After ramping up health infrastructure and although cases were on the rise, the lockdown was ordered to gradually lift in May 2020. Methodology It was a retrospective cross-sectional study of all patients admitted to our medical college trauma center in April 2020 and who were compared with April 2019. We studied the prevalence and outcome of road traffic accidents (RTA), assault, and fall from height (FFH) victims admitted to our trauma center. Data were collected using an Excel sheet, and statistical analysis was done using the latest available SSPS software. Results Our study groups included 124 trauma patients from April 2020 as compared with 322 trauma patients from April 2019. The intense lockdown resulted in a decrease in the number of RTA (-72%), FFH (-29%), and assault patients (-7%). There was no statistically significant change in patients' care and management at our center in terms of mortality and morbidity. No Covid-19-positive neurosurgery trauma patients were admitted in April 2020 to our center. Conclusion During the stringent lockdown period of April 2020, there was a significant decrease in the number of trauma victims, especially RTAs. The intense lockdown resulted in a significant decrease in the proportion of trauma victims under the influence of alcohol, head injuries, fractures, and dislocations along with a decrease in the number of cases of assaults and FFH as compared with the unlock period.

ABSTRACT

Freight vehicle crashes are more serious than regular vehicle crashes because they are likely to lead to major damage and injury once they occur;therefore, countermeasures are needed. The fatality rate from freight vehicle crashes is 1.5 times higher than that of all other accidents, and the death rate from expressway freight vehicle crashes continues to increase. In this study, the ten-freight-vehicle crash severity models (the ordered logit and probit model, the multinomial logit and probit model, mixed-effects logit and probit model, random-effects ordered logit and probit model, and multilevel mixed-effects ordered logit and probit model) are used to analyze the freight vehicle crash severity factors. The model was constructed using data collected from expressways over eight years, and 13 factors were derived to increase the severity of crashes and 7 factors to reduce the severity of crashes. As a result of comparing the 10 constructed models using AIC and BIC, the multilevel mixed-effects ordered probit model showed the best performance. It is expected that it can contribute to improving the safety of freight vehicles in the expressway section by utilizing factors related to the severity of crashes derived from this study.

ABSTRACT

Introduction: Chronic rhinosinusitis with nasal polyposis (CRSwNP) is characterized by a type 2 pattern of inflammation resulting in the production of some cytokines such as interleukin (IL)-4, IL5, and IL13. Options for treatment-resistant CRSwNP include aspirin desensitization, recurrent topic and systemic corticosteroid use, and functional endoscopic sinus surgery (FESS). However, frequent relapses after medical and surgical treatment have been observed. Thus, dupilumab, a human recombinant monoclonal IgG4 antibody, changes radically the treatment of CRSwNP because of its binding effects on major drivers of human type 2 inflammatory processes [1-3]. Considering its recent approval, it may be useful to evaluate its safety profile. Objective: The aim of this study was to describe better adverse drug reactions (ADRs) related to dupilumab in the treatment of CRSwNP analyzing all individual case safety reports (ICSRs) collected into the European Spontaneous Reporting System (SRS) database. Methods: All ICSRs recorded starting from the drug approval up to 31 December 2021 with dupilumab reported as suspected and having the specific indication of CRSwNP were considered. A descriptive analysis was conducted to assess demographic characteristics and dupilumab-related variables. Results: Out of 10,400 ICSRs related to dupilumab, only 481 (4.6%) had CRSwNP indication, of which 68.2% were related to adults and 54.3% to females. The 68.4% were serious;however, ICSRs mainly led to a completely or partial recovering (25.4%) and 8 cases were fatal (1.7%). The time to onset (TTO) of ADRs was 25 (1-84.75) days while the time to resolution (TTR) was 5 (1.75-15.75) days. Analyzing ADRs by System Organ Classes (SOCs), the most reported were general and administration site conditions (36.4%) followed by injuries (21.6%), infections (21.2%), respiratory (19.1%), skin (16.6%), and nervous system disorders (16.4%). Looking at Preferred Terms (PTs), arthralgia (7.3%), eosinophilia (6.9%), COVID-19 (6.0%), pyrexia (5.8%), asthenia (5.6%), rash (5.4%), and dyspnoea (5.2%) were the most reported. The 7.5% of ICSRs described an aggravated condition with persistent nasal polyps: in 4 cases (0.8%) a nasal polypectomy was required. Considering fatal ICSRs, two cases were related to progression of COVID-19, one to road traffic accident, one to accidental death and the others were not fully specified. Conclusion: These results showed that dupilumab-related ICSRs are not commonly reported in CRSwNP. However, given the good treatment response and the minimal adverse effects observed, clinicians should consider treating CRSwNP with dupilumab. Moreover, additional analyses are necessary to better outline the safety profile of dupilumab in this particular setting.
